Background
The Nadler equations estimate total blood volume in adults from height, weight and sex. They were derived from radioisotope dilution measurements of plasma and red-cell volume in healthy adults, producing sex-specific formulae that combine a height-cubed term, a weight term and a constant. Because men and women differ in body composition and blood volume per kilogram, separate equations are used. The result is reported in litres and underpins several transfusion and apheresis calculations.
Interpreting the result
A larger output reflects greater body size, with men generally having a higher estimated blood volume than women of the same height and weight. The estimate feeds calculations such as maximum allowable blood loss, exchange transfusion and plasmapheresis volumes. It assumes a normal adult body habitus, so values should be interpreted with care at extremes of body composition. The figure is an estimate, not a measured volume.
Worked example
For an 80 kg man who is 1.80 m tall, the Nadler equation gives an estimated blood volume of roughly 5.3 L. This estimate can then be used, for example, to calculate the maximum allowable blood loss before transfusion is likely to be needed.

- Accuracy falls at extremes of body habitus, such as severe obesity or very low body weight, where the height-cubed term can distort the estimate.
- The output is an estimate of total blood volume — it does not by itself indicate the patient's volume status or degree of anaemia.
- Ensure height and weight are entered in the correct units (cm and kg), as errors here change the result substantially.
